All information provided to our company or any therapist associated with Key 4 Changes LLC is protected by HIPPA and information will not be shared unless an appropriate ROI (Release of Information) is provided.

Release of Information

If you wish to have any of your information provided to any outside agency/person/provider/company, please contact us in order to ensure you complete the correct form in order for us to do so. Items released can be specifically individualized to provide a release of only the specific information in which you want to share.

Reasons to Breach Confidentiality

In order for one of our therapists or company to release any identifiable information to any agency/person/provider/company without your consent, our provider has concluded that they feel that the individual is suspected of harming themselves or harming others.
